Death Of Nursing Home Patient Attributed To Fall From Mechanical Lift

The oversight of a basic safeguard has been attributed to the death of an 84-year-old nursing home patient who fell from a mechanical lift being used by staff to move her.  An investigation into the incident by the Minnesota Department of Health concluded that two CNA's at the facility identified as Devine Providence Health Canter failed to properly utilize the straps on the lift intended to keep patients safely in place.  While the elderly patient was being moved, she fell head-first from several feet.  After receiving medical attention, the woman's condition continued to deteriorate and she died several weeks later.... Minnesota Nursing Home Blamed for Resident's Choking Death

                                           A recently-released Minnesota State Health Department report revealed new information about a questionable choking death in a Mahnomen nursing home. According to the report, 82-year-old Keith H. Johnson was a resident of the Mahnomen Health Center, when he was served a deadly meal of solid foods on Dec. 13, 2010. The decision to serve solid foods clearly violated Johnson’s doctor’s orders, which said he was only to eat pureed meals. Nursing Home Patient Dies After Receiving 'Toxic' Medication Overdose

Nursing Home Blamed For Resident Choking To Death On Raw Cukes

'Significant Medication Errors' Discovered In Nursing Home Following Investigation Related To Patient Injury & Death

Following a medication-error related death and other medication-related problems that resulted in patient injuries, Minnesota Department of Heath investigators concluded that Fair Oaks Lodge in Wadena, MN was guilty of neglect and other federally mandated nursing home standards.
